What Is PCOS and Why Does It Cause Unwanted Hair?

Polycystic ovary syndrome is a hormonal disorder affecting roughly 6 to 12 percent of women of reproductive age, making it one of the most common endocrine disorders in young women globally. The classical features include irregular or absent menstrual periods, polycystic ovaries on ultrasound, and elevated androgens including testosterone, free testosterone, DHEA-S, and dihydrotestosterone (DHT).

The elevated androgens cause the extra hair symptoms: facial hair growth in masculine patterns (chin, upper lip, jawline, neck, sideburns), as well as hair on the chest, abdomen, lower back, and inner thighs. The medical term for excessive hair growth in androgen-dependent areas in women is hirsutism.

The Androgen Connection: Why PCOS Clients Need More Sessions

As long as your androgen levels remain elevated, dormant follicles can continue to be stimulated into terminal hair growth. Laser destroys the follicles it targets, but new follicles can become activated by ongoing androgen stimulation. This is the source of the more sessions needed plus lifetime maintenance pattern in PCOS clients. It is not a failure of the laser; it is the hormonal terrain.

Managing the underlying hormonal condition makes laser more effective and longer-lasting. Many of our PCOS clients see best results from combining laser hair removal with medical management of their PCOS through their primary care provider or endocrinologist.

Most-Affected Body Areas in PCOS

In our clinical experience treating PCOS clients in NYC, the most-requested treatment areas are: chin and lower face (most-requested), upper lip, sideburns and lateral cheeks, front of neck, chest (particularly periareolar), abdomen (the linea alba and lower abdomen), lower back and lumbar region, and inner thighs.

The reasons: higher follicle density in androgen-dependent areas, coarser and deeper-rooted hair that requires gradual dosing, ongoing follicle activation from elevated androgens, and hormonal cycling that can produce new visible hairs between sessions.

After the initial course, most PCOS clients need 2 to 4 maintenance sessions per year rather than the 1 to 2 that non-PCOS clients need. The maintenance requirement decreases if the underlying PCOS is medically managed.

Hormone Medications and Laser Hair Removal

Most medications used to manage PCOS are compatible with laser hair removal. Spironolactone does not contraindicate laser and does not affect skin response. Oral contraceptives do not contraindicate laser. Metformin does not contraindicate laser. Inositol supplements do not contraindicate laser.

The only medication category that requires significant attention is isotretinoin (Accutane), which requires a 6-month wait after stopping before laser hair removal. Tell us your full medication list at consultation.

Laser hair removal treats the symptom (the hair) but not the cause (the elevated androgens). For best long-term results, we encourage our PCOS clients to be under active medical management for their PCOS with a primary care provider, OBGYN, or endocrinologist.
